Glenrose Rehabilitation Hospital – School Age Neurodevelopmental Assessment & Outreach Clinic

Referral Pathway

  • School-aged children 6 to 18 years of age.
  • Have a query of Autism Spectrum Disorder.
  • Children must have already had the available developmental assessments, (e.g. SLP, OT, Psychology, Audiology) in the community and should have started intervention if appropriate.
  • Children should have been referred for mental health services, if appropriate, prior to making a referral to SNAC.
  • Children should have received treatment for ADHD, if appropriate, prior to making a referral to SNAC.

Completion of a Developmental Pediatric Referral Form by the child’s physician, pediatrician, nurse practitioner or psychiatrist is required.  The referral form with accompanying medical information can be faxed to:  Pediatric Central Intake, Glenrose Rehabilitation Hospital @ 780-735-6293.

Description of Service

Sees children when there is a question of Autism Spectrum Disorder. The clinic identifies problems related to brain development, such as Autism Spectrum Disorder, in school-aged kids who have tried all available community resources. The clinic also:

  • teaches about a child’s skills
  • advises on ways to help kids grow and improve
  • refers for community help, if required
