National Day for Truth & Reconciliation 2024
The EZPCNC is joining AHS in marking the 2024 National Day for Truth & Reconciliation by co-sponsoring a keynote talk by Elder Dr. Francis Whiskeyjack. Events take place on Wednesday, September 25, 2024, and will be open to online and in-person attendees.

AHS welcomes Elder Dr. Francis Whiskeyjack as our keynote speaker. Francis is from the Saddle Lake First Nation and will be sharing his story as a residential school survivor. As a community leader and educator, Francis will also share his thoughts on reconciliation, and the accomplishments that led to him receiving an honorary doctorate degree from the University of Alberta and having an Edmonton public school named after him. His story of loss, strength, resilience and hope will be inspirational and leave a lasting memory for all. This event is sponsored by the Edmonton Zone Primary Care Networks Committee.

Past Events
In collaboration with AHS Primary Health Care Integration Network, we’re hosting online workshops this summer for anyone doing work related to Home to Hospital to Home transitions, focusing on implementation strategies and resources around confirmation of primary care provider, LACE score for transition planning and scheduling follow-up appointments, discharge summary and follow-up to primary care.
Workshops are open to clinic leads, practice facilitators, and quality improvement and clinic staff who support family physicians. Family physicians also welcome.
What did our July participants tell us?
Participants noted that takeaways which they could apply to their own work included:
- Educating clinic teams on the discharge summary formats and the LACE score – particularly ensuring all team members are aware so they can support their team in coming up with ideas for standardized process
