What is a PCN?
Integrated care for all your primary health care needs
PCNS, or Primary Care Networks, follow a team-based health care model. Doctors and healthcare professionals such as nurses, mental health therapists, social workers and dietitians work collaboratively to provide integrated care for all your primary health care needs. PCNs also offer patient education classes and other services. Your family doctor likely belongs to a PCN – you can ask them directly, or find a clinic list below.
